Terms and Conditions of Membership

Membership with Immediate MedCare is subject to the following terms and conditions. “IMC” refers to Immediate

MedCare, LLC and “you” refers to all members of IMC.

1. Your membership requires that all payments are met at the time of your visit including monthly fees and the visit for that day. 72 hour waiting period for new enrollment.

2. You must bring a valid photo I.D. of the member being treated.

3. Falsification or transfer of ID for the intent of obtaining treatment for an unpaid member will result in immediate revocation of membership privileges and a charge for service at the normal rate.

4. Treatment for minors will not be given without a parent or legal guardian present.

5. Rates are subject to change without notice.

Certain situations may require a referral to a specialist or to the hospital. IMC membership does NOT cover these additional expenses and is not insurance.

YOUR MEMBERSHIP INCLUDES:

Simple lacerations suitable for minor emergency care (excluding materials)

Initial screening of motor vehicle accidents

Lab (Urinalysis, Strep screens, mono spots)

Burn (1st and 2nd degree)

Sprains and bruises

Minor Fractures (breaks-excluding material)

School physicals

Sports and camp physicals

YOUR MEMBERSHIP DOES NOT INCLUDE:

Preventative Treatment or Maintenance Care

Physical Therapy of Rehabilitation

Maintenance care for diabetes, thyroid disease, etc.

Routine pregnancy care

Mammograms, MRI, CAT scans, PET scans, Doppler Studies

Intravenous (IV) Nutrition

Life-threatening events such as gunshot wounds and heart attack

Hospitalizations, treatments with other doctors at another facility
